Third Party TRF Pick Up
If you would like someone else to pick up your TRF:
1. Complete the TRF Authorization Form
Download the form here.
2. Email the center
Include your signed TRF authorization form, a copy of your ID, the authorized representative's ID, and the pick-up date and time.

3. Pick up your TRF
Third party must bring a copy of the candidate's ID document, their own original ID document and authorization signed by the candidate.
